Recently, a PCP changed a patient’s psychiatric regimen, and I ain’t gonna lie… I was feeling a type of way. 😂📞 “Dr. Unknown, what are you doing, bro?!” Initially, it felt like a boundary was crossed. But we’re all on the same team, and I appreciate that my patient has another physician looking out for them. Primary care does a LOT of mental health care. An analysis found that primary care providers prescribed nearly 74% of antidepressant and anti-anxiety medication fills. There is such a lack of psychiatrist around the country and it’s a void we may never fill for a long time. So, much love to my PCP colleagues, just keep me in the loop when we’re changing the game plan! 😂 Same patient. Same team. Nathaly Ramirez deserved to watch her daughter grow up. She was pushed from 15 stories high with her 8 month old. The child is in critical condition but Nathaly unfortunately passed away. My heart is with her baby, her family, and everyone grieving this devastating loss. 💔 As someone from the Bronx and a psychiatry resident, this hits close to home. Domestic violence can leave wounds you cannot see, fear, depression, trauma, and a loss of safety that affects survivors and children. Imagine if I believed them when they said I couldn’t. I SOAPed into family medicine and gave it a real chance, but psychiatry was still where I saw my future. The support I initially thought I had changed. Four meetings later, I secured a letter of good standing, not a recommendation. I emailed roughly 70% of psychiatry programs looking for a PGY-2 opening, hoping someone would give me a chance. It was late in the cycle. It was painful. But I kept advocating for myself. One of those emails eventually led to an interview at the program I now call home. Today, I’m a PGY-3 psychiatry resident, grateful for my year in FM and the people who helped me make that transition.
