This clip demonstrates two techniques I talk about frequently. . Since the catheter sits behind the bevel of the needle, it’s still in the wall of the vein when you first see flash. Lowering your angle and advancing 2-6mm will ensure the catheter is all the way in the lumen of the vein, and will limit catheter contact with the vein wall while threading. . Tamponade can help stop blood from flowing out of the catheter after you remove the needle (if you aren’t using “blood control” catheters). . Once you thread the catheter, remove your tourniquet, then as you remove the needle, put pressure down on the vein just above the catheter end. This should occlude the catheter and limit leakage as you attach your extension tubing (kind of like when you put your thumb over the end of a straw it keeps the liquid from coming out). You can also place a stack of sterile gauze under the catheter hub before removing the needle. . . *this model is made of ballistic gelatin and is merely a representation of a vein. The gelatin is softer than human tissue and cannot mimic veins perfectly.* . How to tie a tourniquet! . This is a simple one, but can make all the difference. Too often I see nurses struggling to palpate a vein and find that their tourniquet is barely sitting on top of the patient’s skin. This’ll get you nowhere in a hurry. . The point of a tourniquet is to cut off venous return without effecting arterial supply. It needs to be tight but not TOO tight. My advice is to make it hurt (just a little), and then it’s probably just right. . Place your tourniquet flat behind the patient’s arm, 4-6” above where you want to palpate. Stretch one side tight with your dominant hand, then grab it with your nondominant hand and hold it in place while you stretch the other side tight. Then take that end and tuck it from the top down under the other end. This holds them in place while keeping the tourniquet ends out of your venipuncture site. . To remove the tourniquet, pull straight up on BOTH ends. If you pull just one, the other end can end up smacking you or the patient in the face (ask me how I know 😉). .
