This is exactly what we want to see. 👌 This is an isometric bone-patellar tendon-bone (BTB) ACL reconstruction, and one of the things I’m checking here is how the graft behaves as we take the knee from flexion all the way into full extension. The goal? Consistent graft tension throughout the range of motion with no impingement. In this case, there’s no change in graft tension from flexion to full extension and no impingement. The graft was then tensioned and fixed with the knee in full extension. Good position. Good tension. No impingement. Perfection! 🤌 #RyvesMooreMD #WhySOG #ChooseSOG #ACLReconstruction #BTBGraft #PatellarTendonGraft #ACLSurgery #KneeSurgery #SportsMedicine #Orthopedics

Sometimes getting to the shoulder is a procedure in itself 👀 For this reverse shoulder arthroplasty, I’m using a deltopectoral approach to access the shoulder joint. On the way in, we’re also performing a biceps tenodesis - releasing the long head of the biceps tendon from its attachment and securing it in a new position. Why address the biceps during a shoulder replacement? The tendon can be a source of pain, and depending on its condition and the procedure we’re performing, a tenodesis can be an important part of the overall surgical plan. There are a lot of steps before we ever get to placing the new shoulder components. Each one has a purpose, and each one is part of our bigger goal: relieve pain, restore function, and help our patient get back to using their shoulder with confidence. #RyvesMooreMD #WhySOG #ChooseSOG #ReverseShoulderReplacement #ShoulderArthroplasty #BicepsTenodesis #ShoulderSurgery #SportsMedicine #Orthopedics

Tenotomy, peel, or LTO - there’s more than one way to manage the subscapularis during a reverse shoulder arthroplasty. 👀 My preference here is a subscapularis peel, which you can see in the first part of this video as we continue making our way into the shoulder. And this is where I’m curious what everyone else is doing: Tenotomy? Peel? Lesser tuberosity osteotomy (LTO)? And at the end of the case, are you repairing the subscapularis? Why or why not? In the second half of the video, we move on to the next steps - dislocating the humeral head, removing osteophytes, and performing the humeral head osteotomy as we continue preparing for the reverse shoulder replacement. There can be multiple ways to accomplish the same goal in surgery. Understanding the anatomy, the patient in front of you, and the reasoning behind each decision is what matters. Surgeons - what’s your preferred technique? Let me know below. 👇 #RyvesMooreMD #WhySOG #ChooseSOG #ReverseShoulderArthroplasty #ReverseShoulderReplacement #Subscapularis #ShoulderSurgery #Orthopedics #SurgicalTechnique
