Leber Congenital Amaurosis (LCA) | Genetics, Pathophysiology & Management Leber congenital amaurosis (LCA) is a genetically heterogeneous, early-onset inherited retinal dystrophy, typically presenting in infancy and accounting for a significant proportion of inherited childhood blindness. It is most commonly autosomal recessive and results from pathogenic variants affecting photoreceptor development, the visual cycle, ciliary function, or retinal signaling. 🔬 Genetic basis: Associated genes include CEP290, GUCY2D, RPE65, AIPL1, CRB1, and IQCB1. The specific genetic variant influences disease severity, progression, and potential treatment eligibility. 👁️ Clinical manifestations: Profound visual impairment, nystagmus, sluggish pupillary responses, photophobia or paradoxical eye pressing (oculodigital sign), and variable retinal abnormalities. Some genetic subtypes may involve systemic manifestations. ⚙️ Pathophysiology: Dysfunction of retinal photoreceptors or the retinal pigment epithelium disrupts phototransduction and visual-cycle regeneration, resulting in severely impaired vision from early life. 🧪 Investigations: Comprehensive ophthalmic assessment, fundus examination, optical coherence tomography (OCT), fundus autofluorescence, full-field electroretinography (ERG), and molecular genetic testing. Electroencephalography (EEG) is not a routine LCA investigation but may be indicated if seizures or neurological symptoms suggest an associated disorder. 💊 Management: Low-vision rehabilitation, visual stimulation and developmental support, educational accommodations, genetic counseling, and regular ophthalmic follow-up. Patients with confirmed biallelic RPE65 variants and sufficient viable retinal cells may be eligible for voretigene neparvovec gene therapy. Other treatments depend on the genetic subtype and current clinical evidence. 🔭 Research perspectives: Gene replacement, gene editing, optogenetics, and retinal prosthetic approaches are under investigation for selected inherited retinal disorders. 📚 LCA illustrates the critical relationship between molecular genetics, retinal biology, precision medicine, and emerging gene-based therapies. #LeberCongenitalAmaurosis #InheritedRetinalDisease #OphthalmicGenetics #RetinalDystrophy #GeneTherapy
