Most TMD splints fail for the same reason: they're a single appliance, not a progression. A splint on its own doesn't treat the patient. The diagnosis decides which appliance goes in first, how the bite is taken, and when to move to the next appliance. Here's the Phase 1 framework in 7 steps: 1️⃣ Diagnose: acute or chronic, muscular or intracapsular 2️⃣ Select the appliance that matches the joint 3️⃣ Take a phonetic bite. It's a treatment position, not a final one 4️⃣ Progress: pivot → flat plane → muscular demand 5️⃣ Treat for 8–12 weeks, with recalls every ~2 weeks 6️⃣ Wean once you hit MMI, and always offer it first 7️⃣ Phase 2 only if weaning fails, after 6 months stable, not 3 Step 7 is where restorative cases go wrong. Restore too early and the clicking comes back, along with your crowns. 💾 Save this for your next splint case.
